WhatsApp vs SMS for appointment reminders: the numbers are not close

Clinics have been sending SMS reminders for a decade. The open rates tell a different story than most assume.

SMS appointment reminders became standard practice in Indian clinics sometime around 2015. They were cheap, universal, and — at the time — genuinely effective. In 2025, the landscape looks very different.

The open rate gap

Industry benchmarks now put SMS open rates for transactional healthcare messages at around 30 to 35%. WhatsApp messages, by contrast, consistently achieve open rates above 90% — often within the first five minutes of delivery.

The reason is behavioural. SMS inboxes in India are dominated by OTPs and promotional spam. Most users have learned to treat them as noise. WhatsApp is where actual conversations happen — with family, friends, and increasingly, with services people trust.

Read is not the same as acted upon

Open rate matters, but what clinics actually care about is the no-show rate. A reminder that gets opened but ignored is only marginally better than one that never gets read.

WhatsApp reminders have a structural advantage here: they support two-way conversation. A patient who receives a WhatsApp reminder can reply to confirm, reschedule, or cancel — directly in the same thread. This removes the friction that causes patients to ignore SMS reminders (they would have to call the clinic to make any change).

Clinics using FREMN&apos#s WhatsApp reminder flow typically see a 35 to 45% reduction in no-shows within the first month. The biggest gains come from the reschedule flow — patients who cannot make their slot but would have simply not shown up now cancel in advance, freeing the slot for another booking.

Cost is no longer a differentiator

A common objection to WhatsApp reminders is cost. SMS is cheap; WhatsApp Business API messages carry a per-conversation charge. In practice, the economics invert quickly. A single recovered appointment covers the messaging cost for hundreds of reminders. When you factor in the staff time saved from manual follow-up calls, WhatsApp automation pays for itself many times over.

If your clinic is still on SMS-only reminders, the switch is worth making. The data is unambiguous.
